AlphaFlight — Risk Assessment

Preflight Risk Assessment

PAVE · 1–5 scale

Rate each item 1 to 5, honestly, then total the column. 25–49 is normal, 50–74 elevated, 75 and above high. Any 5 is a No-Go. In the elevated or high range solo flight is not authorized; on a dual flight the CFI rates the same items to transfer some of the risk.

This form is based on the PAVE checklist and common accident causal factors. The list is not exhaustive; as pilot in command you have the ultimate responsibility for the safety of the flight. Cross-check weather against an official briefing before departure.
